Four UniCare medical plan options: The four UniCare Participating Provider Plans available for our employees meet varying needs. An eligible employee is a W2 consultant, who works at least 30 hours a week and has worked for The InSource Group for 60 days of continuous employment. Once these criteria are met, coverage will become effective on the first day of the month following. Monthly premiums are made pre-tax via payroll deduction.

Vision discount plan:

The Vision One Eyecare Program, provided by Cole Vision Corporation, offers UniCare members included with the medical plan plenty of options, including discounts on a wide variety of eyeglasses and contact lenses. Vision One is honored at more than 6,000 locations nationwide. Participating providers include independent optometrists and ophthalmologists in addition to Sears Optical, Target Optical, Pearle Vision, and JC Penney Optical.

Individual dental plan:

UniCare offers the Individual and Family Fee-for-Service Dental Insurance Plan to help keep your teeth healthy and your smile bright. This dental insurance plan gives you the option of going to any dentist you choose and there is no waiting period for eligibility. Dedicated professionals have contracted with UniCare to provide a wide range of dental services such as routine check-ups, cleanings, fillings, crowns and dental surgery.

401(k) employee savings plan:

Participation is voluntary. When it comes to managing your investments, you have a choice between self-direction or professional active management. Contributions to the plan are made on a pre-tax via payroll deduction.
